a. Approve and authorize the Director of Health Services or designee to execute a retroactive Platform License Agreement with Conduent Health Communities Corporation (CHCC) for the provision of customization, maintenance, support, and training for the Healthy Communities Institute Platform System that collects de-identified population health data from multiple sources in the amount of $120,000 for a term effective June 24, 2025 through June 23, 2028;
b. Approve and authorize the Director of Health Services or designee to approve up to three (3) future Amendments that do not exceed 10% ($12,000) of the original Platform License Agreement amount, do not significantly alter the scope of services, and do not exceed a revised maximum amount of $132,000; and
c. Approve and authorize the Contracts and Purchasing Officer or their designee to execute future Amendments, provided they do not exceed an aggregate amount of $200,000 and fall within the signing authority of the Contracts and Purchasing Officer.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Director of Health Services or designee to execute a retroactive Platform License Agreement with Conduent Health Communities Corporation (CHCC) for the provision of customization, maintenance, support, and training for the Healthy Communities Institute Platform System that collects de-identified population health data from multiple sources in the amount of $120,000 for a term effective June 24, 2025 through June 23, 2028;
b. SUMMARY:
Conduent Healthy Communities Corporation (CHCC) hosts a Standalone Platform (CCHC Platform), a website template system that collects de-identified population health data from multiple sources. This platform includes a proprietary web-based, customizable data visualization and mapping tool that enables online analytics and data dashboarding by aggregating information such as health and social factors impacting population health in the community.
DISCUSSION:
The customization and implementation of the CCHC Platform will enable the County and community partners to develop targeted strategies, implement programs, and monitor outcomes using de-identified health and social data. This tool supports the Health Department’s efforts to maintain national public health accreditation and enhances transparency by allowing the community to access information directly. The platform also provides visualizations that highlight associations between social, environmental, and behavioral factors that influence community health.
The CCHC Platform allows the Health Department to facilitate community partnerships and support governing entities by presenting meaningful data that combines clinical and community information. By centralizing and organizing publicly available data, the solution addresses the challenge of siloed and disparate systems, which often hinder coordinated engagement with community stakeholders and alignment of resources.
Data to be included in the platform will consist of existing datasets provided by CHCC as well as additional datasets uploaded by Monterey County Health Department (MCHD) staff. All data uploaded for analytics will be de-identified, and the website will remain publicly accessible. Importantly, there is no need for local software installation, as MCHD staff will upload and manage dashboards directly through the platform.
Given its ability to support data-informed decision-making, strengthen community engagement, and improve transparency, it is the recommendation of the Director of Health to approve this agreement with Conduent Healthy Communities Corporation.
The recommended action is retroactive due to lengthy contract negotiations, which extended past the original timeline for approval.
OTHER AGENCY INVOLVEMENT:
County Counsel, Information Technology, and Auditor-Controller have reviewed and approved this Platform License Agreement as to legal form, technology form, and fiscal provisions respectively. Risk Management cannot approve due to the non-standard terms.
FINANCING:
Funding for this agreement is included in the Health Department’s (HEA014-8438) FY 2025-26 Adopted Budget, provisions will be made in the Requested Budget for the appropriate year.
