Report

RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. Approve and authorize the Director of the Department of Social Services or designee to sign an agreement with Unity Care Group for Transitional Housing Placement-Plus and Housing Assistance for eligible former foster and probation youth for the period of July 1, 2026 through June 30, 2027, in the amount of $869,933; and                                       

b. Authorize the Director of the Department of Social Services or designee to sign up to three amendments to this Agreement where the total amendments do not exceed 10% ($86,993) of the amended contract amount, do not significantly change the scope of work, and do not exceed the maximum aggregate amount of $956,926

 

SUMMARY:
Transitional Housing Placement-Plus (THP-Plus) is a provider-based transitional housing placement option for former foster and probation youth ages 18 to 25 who exited foster care at or after age 18.

DISCUSSION:
The goal of THP-Plus is to provide a safe living environment, for up to 36 months, while assisting youth in developing the life skills necessary to be successful living independently.  The Housing Assistance program provides housing navigator services and direct financial housing supports to young adults in Monterey County ages 18-25 who are either homeless or at-risk of homelessness, which priority given to former foster or probation youth.  The goal of the Housing Assistance program is to reduce homelessness among transitional-age youth in Monterey County.

 

The purpose of this agreement is to contract with Unity Care Group to provide Transitional Housing Placement - Plus Program services for former foster and probation youth.  The monthly participant rate is three thousand, four hundred seventy-two dollars ($3,472) per participant at a maximum of (10) participants per month.  In addition, the purpose of this agreement is to contract with Unity Care Group to serve at least 24 youth in both the Housing Navigator and Housing Assistance programs in Monterey County during the fiscal year. 

 

OTHER AGENCY INVOLVEMENT/COMMITTEE ACTIONS:
The Office of County Counsel has reviewed and approved the Agreements as to form, as has the Auditor-Controller’s Office as to financial provisions.

 

FINANCING:
This agreement is funded with State THP2 funding, California Department of Housing and Community Development funds, and Social Services Realignment.  Sufficient appropriations and estimated revenues for SOC001 are included in the FY 2026-27 Recommended Budget.

BOARD OF SUPERVISORS STRATEGIC PLAN GOALS:
This agreement correlates to the Well-Being and Quality of Life Strategic  Plan Goal adopted by the Board of Supervisors by providing Housing and Supportive Services for high-needs former foster youth.
