RECOMMENDATION:
 
It is recommended the Board of Supervisors authorize the Purchasing Manager for Natividad Medical Center (NMC) to execute Amendment No. 5 to the Agreement (A-11829) with PSM Medical Imaging Specialists Inc. for Information Technology Picture Archiving and Communication System (PACS) Project Management & Implementation Services at NMC, extending the Agreement from April 1, 2010 through June 30, 2015 and adding $366,168 for a revised total Agreement amount not to exceed $1,946,971.
      
SUMMARY/DISCUSSION:
      
In October 2010, NMC installed the first phase of a Picture Archiving and Communication System (PACS) which provides digital imaging for the Diagnostic Imaging Department.  PSM Medical Imaging Specialists, a locally recognized expert in PACS and imaging software, has provided project management and onsite support services for the NMC PACS implementation. The first phase of the PACS implementation included installing and implementing a PACS system to transition from analog to digital imaging in the Diagnostic Imaging Department.  In the first phase we completed the transition of CAT Scan, MRI, Ultrasound, and X-ray modalities. The next phase will include many enhancements to the current infrastructure and add more applications to improve the functionality of the digital system. The proposed services and projects included in this amendment are listed below.
 
This one year amendment will allow for the following:
·     Project management support and resources for the implementation of McKesson Pacs Software upgrade to version 12.0 and enhancement options to increase the functionality for users.
·     Project management and support to move the main frame server to a virtual data base.
·     Offsite image distribution services
·     Oversight of the PACS Information Technology infrastructure (PACS Administrator)
·     Project management and support of PeerVue, the Quality Assurance system, in the Diagnostic Imaging Department.
 
OTHER AGENCY INVOLVEMENT:
 
County Counsel has reviewed and approved this Amendment No. 5 as to legal form and risk provisions. Auditor-Controller has reviewed and approved this Amendment No. 5 as to fiscal provisions. The CAO-Budget and Analysis Division has advised to inform the BOS the spending authority (appropriations) associated with NMC's FY 2014 Adopted Budget has been exceeded and approval and authorization for modification of NMC's appropriation budget should be presented to the BOS. Since NMC is an Enterprise Fund, this does not prevent continued expenditures, but it is considered good public policy, transparency and fiscal management. The Amendment has also been reviewed and approved by Natividad Medical Center's Finance Committee 5.22.14 and Board of Trustees 6.6.2014.
 
FINANCING:
 
The cost for this Amendment No. 5 is $366,168 for a revised total Agreement amount not to exceed $1,946,971. $366,168 is included in the Fiscal Year 2014/2015 Recommended Budget. NMC is confirming its appropriations position and if its spending authority has been exceeded will return to the Board. There is no impact to the General Fund.
