RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health Services or designee to execute a Mental Health Services Agreement with Felton Institute for the provision of prevention and recovery in early psychosis services for a term retroactive to July 1, 2023 through June 30, 2024, and a total Agreement amount not to exceed $1,046,512 for the term of the Agreement.

 

SUMMARY/DISCUSSION

Felton Institute, formerly Family Service Agency of San Francisco dba Felton Institute, is a non-profit agency providing the (re)MIND program for the provision of prevention and recovery in early psychosis services in Monterey County. The (re)MIND program is an evidence-based treatment that identifies psychosis at the earliest possible point to reduce the impact of psychosis on the individual and their family.  (Re)MIND has shown to have dramatic improvement reducing crises, avoiding hospitalizations, improving functional capacity, promoting school/workforce participation, and engaging youth and families.

 

This Agreement, in its entirety, is being presented to the Board of Supervisors for retroactive approval for several reasons. This Agreement was originally brought to the Board on June 27, 2023, through Delegated Authority because Fiscal Year 2022-23 was the first year in which the Department of Health Care Services implemented the CalAIM Payment Reform, which among other changes, introduced a new billing structure that posed a significant learning curve for certain contractors, notably Felton Institute. The authorized Agreement amount was $723,410, however, there was an unforeseen shortfall in the Felton’s service delivery projections due to the intricacies of the new billing procedures. Furthermore, efforts to conclude the contract were hindered by the vendor's lack of responsiveness until March 2024, during which it was revealed that they had exceeded the negotiated contract amount.

 

This Agreement contains the County’s standard 30-day “no cause” provision (Section V, B) and an additional defunding provision (Section V, D), which provides the County the ability to amend or terminate the Agreement in the event of a reduction and/or termination of funding.

 

This work supports one of the County of Monterey Health Department’s 2018-2024 Strategic Plan Goal(s): 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one of the ten essential public health services, specifically: 7. Link people to needed personal health services and assure the provisions of health care when otherwise unavailable.

OTHER AGENCY INVOLVEMENT:

The Office of County Counsel and Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions, respectively.

 

FINANCING:

This Agreement is funded by Federal Financial Participation (43%), Mental Health Services Act Prevention and Early Intervention (50%); and SAMHSA Block Grant funds (7%). The funds for FY 2023-24 have been included in the Health Department’s Behavioral Health Bureau (HEA012, Unit 8410) FY 2023-24 Approved Budget.
