Title: a. Title

RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.                     Approve and authorize the Director of Health or designee to sign Agreement #14-10959 with the California Department of Public Health in the amount of $1,000,000, for the period January 1, 2015 to June 30, 2018, to provide services to local communities with populations at high risk for diabetes and cardiovascular disease under Domains 3 and 4, Health Systems Interventions of the 1305 Supplemental Funds in partnership with area safety net providers; and

b.                     Authorize the Director of Health or designee to approve three future amendments up to ten percent (10%) of the original contract amount, which does not significantly alter the scope of services.

c.                     Authorize the Auditor-Controller to amend Fiscal Year 2014-15 Adopted Budget Fund 001, Appropriation Unit HEA014, Unit 8438 to increase revenue and appropriations in the amount of $250,000. 

 

SUMMARY/DISCUSSION:

Monterey County Health Department was approached by the California Department of Public Health (CDPH) to be one of four counties to receive a 1305 Supplemental Funds four-year grant to support cross-cutting approaches to promote health and prevent and control chronic disease and associated risk factors effective January 1, 2015.

 

The grant funds will be used to support work to:

1)   Implement quality improvement processes in health systems through assessments and planning to increase:

a) Electronic health records adoption and the use of health information technology to improve performance,

b) The institutionalization and monitoring of aggregated/standardized quality measures at the provider and system level, and

c) Engagement of non-physician team members in hypertension and diabetes in health care systems.

2)   Plan for and conduct training for local health system for expansion of community-clinical linkages (ensuring that communities support and clinics refer patients to programs that improve chronic disease conditions) through assessments, planning, and training to increase:

a) Access, referral and reimbursement for lifestyle change programs for the prevention of type 2 diabetes,

b) Engagement of community health workers in the provision of self-management programs, and

c) Engagement of community health workers to promote linkages between health systems and community resources for adults with high blood pressure.

 

Health Department staff has been working with CDPH staff for several months to finalize the Agreement.  Unfortunately, there were unforeseen delays experienced by CDPH in finalizing the Agreement and Health was not in receipt of final Agreement until June 2, 2015. The Health Department seeks approval to sign Agreement retroactive to January 1, 2015.   

 

This work supports the Monterey County Health Department's 2011 - 2015 Strategic Plan initiatives: 1) Empower the community to improve health through programs, policies, and activities and 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one or more of the ten essential public health services, specifically: 3) Inform, educate, and empower people about health issues and 4) Mobilize community partnerships and action to identify and solve health problems.

 

OTHER AGENCY INVOLVEMENT:

County Counsel, Risk Management, and the Auditor-Controller have reviewed and approved this Agreement as to legal form, liability, and fiscal provisions respectively.  A copy of the Agreement is on file with the Clerk of the Board.

 

FINANCING:

This Agreement is fully funded by the California Department of Public Health.  The Health Department also requests an amendment to the Fiscal Year (FY) 2014-15 Health Department Administration Bureau (001-4000-HEA014-8438) Adopted Budget to increase estimated appropriations and revenue in the amount of $250,000.  Although this grant is not specifically included in the Department's FY 2015-16 Administration Bureau (001-4000-HEA014-8438) Recommended Budget, there are sufficient appropriations and revenue included.
