Approve and authorize the Director of Health or the Assistant Director of Health to execute Amendment No. 1 to Agreement A-14442 with Sun Street Centers for the provision of Whole Person Care Access Services for case management, transportation, and accompaniment, of Whole Person Care (WPC) enrollees to medical, mental health, social services, and housing services, as coordinated by County to increase the total Agreement by $4,554 for a total Agreement not to exceed $211,055, for the term July 1, 2019 through December 31, 2020.

RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health or the Assistant Director of Health to execute Amendment No. 1 to Agreement A-14442 with Sun Street Centers for the provision of Whole Person Care Access Services for case management, transportation, and accompaniment, of Whole Person Care (WPC) enrollees to medical, mental health, social services, and housing services, as coordinated by County to increase the total Agreement by $4,554 for a total Agreement not to exceed $211,055, for the term July 1, 2019 through December 31, 2020.

 

SUMMARY/DISCUSSION:

The County of Monterey Health Department (“Health”) administers the WPC program that coordinates physical health, behavioral health, and social services for vulnerable Medi-Cal beneficiaries who are high users of multiple systems and continue to have or are at risk of poor health outcomes. WPC services must additionally ensure that WPC enrollees receive crucial social-emotional support and advocacy while they attempt reintegrate with health care providers, housing, and employment. The services to be provided through this agreement are (1) working collaboratively and at the direction of WPC staff to provide case management, (2) providing transportation and accompaniment for WPC enrollees to health and social services appointments or other agencies and organizations as requested by WPC staff, and (3) assisting enrollees with the completion of applications and registrations to facilitate their access to housing, social service benefits, training, and employment.                                                                           

 

This work supports the Monterey County Health Department 2018-2022 strategic plan initiatives: 1.  Empower the community to improve health through programs, policies, and activities; and 2.  Enhance community health and safety by emphasizing prevention. It also supports one or more of the ten essential public health services, specifically:  4. Mobilize community partnerships and action to identify and solve health problems.  7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable. 

 

OTHER AGENCY INVOLVEMENT:

The office of County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to form and fiscal provisions, respectively.

 

FINANCING:

There is no impact to the General Fund resulting from the approval of this action.  There are sufficient appropriations in the Health Department’s (8438-HEA014) FY 2020-21 Adopted Budget.
