Title: a. RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.                     Ratify execution by the Outpatient Services Manager, as designee of the Director of Health, of a three-year Patient-Centered Medical Home (PCMH 2014) Recognition Program Agreement with the National Committee for Quality Assurance (NCQA) with a retroactive effective date of January 23, 2017, submitted in connection with an application to NCQA (“NCQA Application”) seeking renewal of NCQA’s recognition of the Laurel Internal Medicine Clinic as a Patient Centered Medical Home; and

b.                     Ratify execution by the Director of Health of a Business Associate Agreement (BAA) with NCQA, with an effective date of February 1, 2017 and which shall remain in effect for the term of the PCMH 2014 Recognition Program Agreement; and

SUMMARY/DISCUSSION:

The Health Department Clinic Services Bureau (Clinic Services) operates nine community clinic sites designated as Federally Qualified Health Center Look-Alikes. Under the direction of the County and governance of the Community Health Center Board, the clinics provide preventative, primary, and specialty medical care services. 

 

In 2011, the Laurel Internal Medicine Clinic was recognized by NCQA as a Patient-Centered Medical Home (PCMH).  PCMH is a model of care that emphasizes care coordination and communication to transform primary care into a team-based care delivery “home” for patients. NCQA PCMH Recognition is the most widely adopted model for transforming primary care practices into medical homes. Research confirms medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care. 

 

For the Laurel Internal Medicine Clinic to renew its PCMH recognition, a Patient-Centered Medical Home Recognition Program Agreement (Agreement) and Business Associate Agreement (BAA) was signed by the County and submitted with the application in February.  Due to time constraints, the Agreement was signed by the Outpatient Services Manager and the BAA was signed by the Director of Health with the understanding that the document be presented to the Board of Supervisors for ratification.  The Agreement outlines the terms for participation in PCMH, while the purpose of the BAA is to satisfy certain standards and requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Rules.  

 

This work supports the Monterey County Health Department 2011 - 2015 Strategic Plan initiative to (3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports one or more of the ten essential public health services, specifically: (7) Link people to needed personal health services and assure the provision of health care when otherwise unavailable; and (9) Evaluate effectiveness, accessibility, and quality of personal and population-based health services.

 

OTHER AGENCY INVOLVEMENT:

County Counsel and Health IT have reviewed the Agreement and BAA as to legal form and privacy provisions.

 

FINANCING:

There are no fiscal provisions included in the Agreement or BAA.
