RECOMMENDATION:

It is recommended that the County of Monterey Board of Supervisors:

a.  Authorize the Director of Health Services or designee to execute Amendment No. 3, effective upon execution, to a Platform License Agreement, No. A-14686, between the County of Monterey and Conduent Health Communities Corporation (CHCC),  adding a Data Usage Agreement; and

b.  Approve and authorize the Health Director Services or designee to approve up to three (3) future Amendments that do not exceed 10% ($7,000) of the original Platform License Agreement amount, do not significantly alter the scope of services, and do not exceed a revised maximum amount of $187,000.

 

SUMMARY/DISCUSSION:

Conduent Healthy Communities Corporation (CHCC) hosts a Standalone Platform (CHCC Platform) which is a customizable website template system that collects and reports de-identified population health data from multiple sources.  

 

The customization and use of CHCC Platform enables the county and community partners to develop focused strategies, implement programs, and monitor outcomes using the data while maintaining national health accreditation.   It will also enable the community to access the information which improves transparency and helps to visually draw associations between social, environmental, and behavioral factors that impact health.  

 

The execution of Amendment No, 3 will enable MCHD to include a Data Use Agreement for non-public patient level data, Health and Safety Code Section 128766, for the provision of CHCC to utilize the data collected by County from California Department of Health Care Access and Information (HCAI) limited patient-data sets to include additional data analytics deidentified hospitalization, emergency room, and mental health data to the existing platform license, and continue to provide tools and templates for data visualization and mapping that will be publicly accessible through June 23, 2025.

 

No installation of software locally for MCHD staff is required for data upload, use, or customization of the website.  MCHD Staff will utilize the data and website template to provide publicly accessible information on:  <https://www.datasharemontereycounty.org/>.

 

This work supports the Monterey County Health Department 2018-2024 Strategic Plan Goals: 1. Empower the community to improve health; and 2. Enhance community health and safety through prevention. It also supports one of the ten essential public health services, specifically: 9. Evaluate effectiveness, accessibility, and quality of personal and population-based health services.

 

OTHER AGENCY INVOLVEMENT:

The Offices of the County Counsel and Auditor-Controller have reviewed and approved this Data Usage Agreement as to form and fiscal provisions, respectively.

 

FINANCING:

Funding for this agreement is included in the Health Department’s (HEA014-8438) FY 2023-24 Adopted Budget and provision for it will be made in the Department’s Requested Budget for the years out.
