a. Approve and authorize the Director of Health Services or designee to execute Modification No. 1 to agreement No. A-15579 the Coordination Agreement for Cardiac Arrest Registry to Enhance Survival (CARES) Services for the provision of cardiac arrest data between the County of Monterey and the County of Sonoma for a period retroactive to June 30, 2026 through June 30, 2031 and replace Sections 4.1 (Term and Termination) and 7.6 (Notices) with updated Sections 4.1 and 7.6; and
b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future amendments that do not significantly change the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health Services or designee to execute Modification No. 1 to AGREEMENT No. A-15579 the Coordination Agreement for Cardiac Arrest Registry to Enhance Survival (CARES) Services for the provision of cardiac arrest data between the County of Monterey and the County of Sonoma for a period retroactive to June 30, 2026 through June 30, 2031 and replace Sections 4.1 (Term and Termination) and 7.6 (Notices) with updated Sections 4.1 and 7.6; and
b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future amendments that do not significantly change the scope of services.
SUMMARY:
The Director of Health Services seeks approval to execute Modification No. 1 to the Coordination Agreement for Cardiac Arrest Registry to Enhance Survival (CARES) Services with the County of Sonoma. This amendment extends the Agreement from June 30, 2026, to June 30, 2031. The amendment also replaces Section 4.1 and 7.6 with updated Sections 4.1 and 7.6.
DISCUSSION:
The Health Department’s Emergency Medical Services (EMS) Bureau is designated by the County of Monterey Board of Supervisors as the local Emergency Medical Services Agency (LEMSA), pursuant to California Health and Safety Code, Section 1797.200. The EMS Agency is responsible for planning, implementing, and evaluating the local EMS system throughout the County of Monterey. In addition, Pursuant to California Code of Regulations, Title 22, Section 100253, the local EMS agency is responsible for developing, implementing, and overseeing, in cooperation with other EMS system participants, a system-wide EMS Quality Improvement (QI) program.
CARES is a national registry for the collection and reporting of data on patients suffering from cardiac arrest. CARES Registry compiles a detailed report specific to local EMS systems and provides the information to Local EMS Agencies (LEMSAs) that have entered into a contract to receive the data. The report provides statewide and national reports for comparison to similar systems, and data related to topics such as bystander CPR, survival statistics, and demographics for this group of patients. This Amendment will assist the EMS Agency in continuing to meet its responsibilities related to developing, implementing, and overseeing the system-wide EMS Quality Improvement (QI) program. The reports assist the EMS Agency in the monitoring of indicators related to clinical care and patient outcome.
Modification No. 1 includes the following changes:
• Extends the Agreement for a term retroactive to June 30, 2026, through June 30, 2031.
• Replaces the existing Section 4.a (Term and Termination).
• Replaces the existing Section 7.6 (Notices).

OTHER AGENCY INVOLVEMENT:
The Office of the County Counsel has reviewed and approved this agreement as to legal form.
FINANCING:
There is no impact on the General Fund associated with this action.
This amendment does not modify the financial obligation between the Counties of Monterey and Sonoma.
