RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. Accept a grant award in the amount of $2,500,000 from the California Department of Health Care Services California Advancing and Innovating Medi-cal (CalAIM) Providing Access and Transforming Health (PATH) Justice Involved (JI) Round 3 grant program;

b. Ratify execution by the Acting Chief Probation Officer of the Acknowledgement of Grant Terms and Conditions, an express condition of the County’s receipt of grant funds from the DHCS CalAIM PATH JI Round 3 grant program, to support State mandated Medi-Cal justice-involved reentry services; and

c. Authorize the Acting Chief Probation Officer to sign any agreement, amendments or extensions, and documentation required by the Grantor after review and approval by the Office of County Counsel.

 

SUMMARY:

The Probation Department has been awarded State funds in the amount of $2,500,000 from the Department of Health Care Services California Advancing and Innovating Medi-cal (CalAIM) Providing Access and Transforming Health (PATH) Justice Involved (JI) Round 3 grant program in support of mandated Medi-Cal justice involved reentry services effective upon grant award through April 1, 2026.

 

DISCUSSION:

On January 1, 2022, California Department of Healthcare Services (DHCS) launched a multi-year DHCS initiative, CalAIM, to improve the quality of life and health outcomes of the Medi-Cal managed care population through the implementation of broad delivery system, program and payment reform across the Medi-Cal program. The PATH Justice Involved Reentry Initiative Capacity Building Program provides funding to support the planning and implementation of the provision of targeted pre-release Medi-Cal services to individuals in state prisons, county jails and youth correctional facilities.  All correctional facilities are mandated to implement Medi-Cal reentry services by March 31, 2026.

 

The Probation Department will be utilizing these funds upon completion of the required implementation plan to support justice-involved processes for youth in the delivery of Medi-Cal reentry services and coordination. This will include collaborative planning with Behavioral Health, Department of Social Services, County stakeholders and other agencies to effectuate successful  pre-release processes as mandated by the State.

 

OTHER AGENCY INVOLVEMENT:

The Office of County Counsel and the Auditor-Controller have reviewed the report as to legal form and fiscal provisions, respectively.

 

FINANCING:

Funding in the amount of $2,500,000 has been awarded to Monterey County effective upon grant award through April 1, 2026.

 

There are currently no projected program expenditures during FY 24-25.  The funding will be incorporated in subsequent fiscal years during the County annual budget process. There is no additional cost to the County General Fund with the recommended action.

This recommendation supports key objectives of: 1) Administration with efficient and effective fiscal management of State funds, and 2) Health and Human Services by providing access to health,  rehabilitative and reentry services to juvenile offenders.
