a. Approve and authorize the Director of Health to sign a three fiscal year term Mental Health Services Agreement (July 1, 2014 to June 30, 2017) with Beverly Health Rehabilitation Services, Inc. dba Golden Living Center San Jose for the provision of mental health services for Monterey County adult residents with severe psychiatric disabilities in the amount of $943,781 for Fiscal Year (FY) 2014-15, $986,866 for FY 2015-16, and $1,031,991 for FY 2016-17 for a total Agreement amount not to exceed $2,962,638; and
b. Authorize the Director of Health to sign up to three future amendments to this Agreement where the amendments do not exceed ten percent (10%) of the original Agreement amount and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health to sign a three fiscal year term Mental Health Services Agreement (July 1, 2014 to June 30, 2017) with Beverly Health Rehabilitation Services, Inc. dba Golden Living Center San Jose for the provision of mental health services for Monterey County adult residents with severe psychiatric disabilities in the amount of $943,781 for Fiscal Year (FY) 2014-15, $986,866 for FY 2015-16, and $1,031,991 for FY 2016-17 for a total Agreement amount not to exceed $2,962,638; and
b. Authorize the Director of Health to sign up to three future amendments to this Agreement where the amendments do not exceed ten percent (10%) of the original Agreement amount and do not significantly change the scope of work.
SUMMARY/DISCUSSION:
The County currently has Agreement A-11979 with Beverly Health Rehabilitation Services, Inc. dba Golden Living San Jose which will expire on June 30, 2014. Beverly Health and Rehabilitation Services, Inc. dba Golden Living Center San Jose is a Skilled Nursing Facility, Institution for Mental Disease for adults with psychiatric impairments and disabilities that require a locked skilled nursing level of care. Treatment services include individualized program to meet the specific needs of each client and a structured regimen with individualized services to assist clients in their development of new skills. Approval of this Agreement meets the requirements of the State Welfare and Institutions Code of Regulations Titles 9 and 22, and sustains the continuity of care services to seriously mentally ill clients.
This work supports the following Monterey County Health Department 2011-2015 strategic plan initiative: 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one of the ten essential public health services, specifically: 7) Link people to needed personal health services and assure the provisions of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
The County Counsel and Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions. A copy of this Agreement is on file with Clerk of the Board.
FINANCING:
This Agreement is funded by the Realignment (100%) revenues. The funds for this Agreement are included in the Health Department's Behavioral Health Bureau (HEA012, Unit 8410) Fiscal Year 2014-15 Recommended Budget.
