a. Approve and authorize the Director or designee of the Department of Social Services to sign an agreement with Health Projects Center for Respite and Aging in Place services to family caregivers for the period of September 10, 2024 to March 31, 2026, in the amount of $624,401; and
b. Authorize the Director or designee of the Department of Social Services to sign up to three amendments to this Agreement where the total amendments do not exceed 10% ($62,440) of the amended contact amount, do not significantly change the scope of work, and do not exceed the maximum aggregate amount of $686,841.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director or designee of the Department of Social Services to sign an agreement with Health Projects Center for Respite and Aging in Place services to family caregivers for the period of September 10, 2024 to March 31, 2026, in the amount of $624,401; and
b. Authorize the Director or designee of the Department of Social Services to sign up to three amendments to this Agreement where the total amendments do not exceed 10% ($62,440) of the amended contact amount, do not significantly change the scope of work, and do not exceed the maximum aggregate amount of $686,841.
SUMMARY/DISCUSSION:
This agreement provides State of California, Department of Aging, Modernizing Older Californians Act Supportive Services to older adults and individuals with a disability in Monterey County. These Supportive Services include the following:
Caregiver Respite, In-Home Personal Care: This service includes the provision of care receiver assistance with eating, bathing, toileting, transferring, and/or dressing (along with care receiver supervision and related homemaker assistance) by a skilled provider.
Caregiver Respite, Home Chore: This service includes a skilled provider or volunteer assisting a caregiver with heavy housework, yard work, and/or sidewalk and other routine home maintenance (but not structural repairs) associated with caregiving responsibilities.
Aging in Place: This new concept helps reduce injuries and allows older adults to remain safely in their homes, resulting in significant cost savings for health care insurers, families, and public agencies. Services include participant enrollment, home assessments, consultations, referrals, client satisfaction surveys, and coordination of building contractor services. These services provide eligible seniors with medical/home modification equipment such as grab bars, nonskid surfaces, shower chairs, raised toilet seats, bedside commodes, and transfer benches, enabling them to age in place safely within their homes.
OTHER AGENCY INVOLVEMENT:
The Office of County Counsel has reviewed and approved the Agreements as to form, as has the Auditor-Controller’s Office as to financial provisions.
FINANCING:
Funding is provided by the State of California, Modernizing Older Californians Act. Sufficient appropriations and estimated revenues of $377,944 are included in the Area Agency on Aging 001-5010-SOC10 FY 2024-25 Adopted Budget and will incorporate the remaining balance of $246,457 in the upcoming budget process.
BOARD OF SUPERVISORS STRATEGIC INITIATIVES:
This Agreement correlates to the Health & Human Services Strategic Initiative adopted by the Board of Supervisors by working towards the goals of improving health and quality of life outcomes by providing nutrition and health promotion services to seniors in Monterey County.
