RECOMMENDATION:
It is recommended that the Budget Committee of the Board of Supervisors support the request to:
a. Amend Fiscal Year (FY) 2014 Health Department Budget 4000-HEA003-8124 to add 2.0 FTE Public Health Nurse III positions; delete 1.0 FTE Secretarial Assistant; delete 1.0 FTE Supervising Public Health Epidemiologist position;
b. Amend Fiscal Year (FY) 2014 Health Department Budget 4000-HEA004-8121 to reallocate 6.0 FTE filled Office Assistant II positions to 6.0 FTE Office Assistant III positions and delete a vacant 1.0 FTE Office Assistant;
c. Amend Fiscal Year (FY) 2014 Health Department Budget 4000-HEA003-8124 to add 1.0 FTE Public Health Nurse III; and  
d. Authorize the County Administrative Office to reflect approved changes in the Fiscal Year 2014-15 Adopted Budget.  
 
SUMMARY/DISCUSSION:
Amend Personnel Policies and Practices Resolution (PPPR) No. 98-394 and Appendix A to create the classification of Assistant Director, Public Health Laboratory, with the salary range as indicated in the attached Resolution.  The Public Health Laboratory plays a critical role in the protection of public health and safety.  The Public Health Laboratory provides local testing to protect the community against public health threats such as infectious diseases (such as ebola and measles), toxins, and biohazards.  Local health professionals, clinics, and hospitals rely on the Laboratory to test and identify substances that could pose a serious risk to public health.  The Laboratory does not currently have a supervisory or operations manager-level position, other than the Laboratory Director.  The Public Health Laboratory requires an additional level of management to meet the vigorous Federal and State requirements of management and supervision in a Public Health Laboratory.  The creation of this proposed classification is designed to meet the vigorous Federal and State requirements and will establish a career ladder to grow potential Director, Public Health Laboratory candidates.  The recommended salary for this proposed classification of Assistant Director, Public Health Laboratory is $6,420 - $8,764/month. As a result of the proposed additional classification of Assistant Director, Public Health Laboratory, a Salary Survey was conducted for the position of Director, Public Health Laboratory to allow for a salary differential that is warranted for the increased complexity and liability of this position.  As a result of this effort, the proposed salary for Director, Public Health Laboratory will increase approximately 16.39% for a new salary range of $8,077 - $11,025/month.
 
This is a request to add 2.0 FTE Public Health Nurse III positions to Regional Health Teams; delete 1.0 FTE Secretarial Assistant (position 80A300001) from the Regional Health Teams; and delete 1.0 FTE Supervising Public Health Epidemiologist (50N220001) from the Communicable Disease program. Regional Health Teams has been unable to fill or keep filled a third Supervising Public Health Nurse (SPHN) despite assertive continuous recruitment efforts. During the past two years duties associated with the third SPHN position have been covered initially by the Public Health Program Manager/Director of Nursing until that position became vacant and the duties shifted to the two existing SPHNs.
 
Add 1.0 FTE Public Health Nurse III (PHN III) to the Public Health Bureau's Maternal, Child and Adolescent Health (MCAH) Program. This position and its scope of work was included in Monterey County's FY15 MCAH Annual Funding Application and subsequently approved the California Department of Public Health, Maternal, Child and Adolescent Health Branch.  The MCAH Branch within the California Department of Public Health establishes the required FTE for each County's MCAH Director based on county population.  Monterey County is required to assign 1.0 FTE MCAH Director; this county utilizes a Supervising Public Health Nurse (SPHN) to perform the duties of the MCAH Director.  However, this SPHN oversees non-MCAH-funded programs that serve the maternal and child health population, which reduces actual time spent on MCAH Director activities. To meet the County's required MCAH Director FTE, the State allows for a portion of the Director's responsibilities to be delegated to a designated MCAH Coordinator.  This PHN III position will be assigned .25 FTE MCAH Coordinator and .75 FTE Medically Vulnerable Infants and Children's Care Coordination Program coordinator; a service component included in the FY 15 MCAH Scope of Work.
 
Request to reallocate 6.0 FTE filled Office Assistant II allocations to 6.0 FTE Office Assistant III allocations in Unit 8121 (positions 80E210001, 80E210002, 80E210003, 80E210004, 80E210006, and 80E210007); and eliminate a vacant 1.0 FTE Office Assistant II allocation in Unit 8121 (position 80E210005). The assigned duties of three filled Office Assistant II allocations in the California Children's Services (CCS) Administrative program, two filled Office Assistant II allocations in the CCS Medical Therapy Program, and one filled Office Assistant II allocation in the Child Health and Disability Prevention (CHDP) Program have been determined to meet Office Assistant III specifications.  This request reallocates these positions from the Office Assistant II to Office Assistant III classification.
 
This action will support the Monterey County Health Department's 2011-2015 Strategic Plan Initiatives: 1. Empower the community to improve health through programs, policies, and activities; 2. Enhance community health and safety by emphasizing prevention; 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.
 
OTHER AGENCY INVOLVEMENT:
The Human Resources Department has reviewed and concurs with this recommendation.
 
FINANCING:
The cost of the requested positions is $69,905 for Fiscal Year 2014-15 and will be offset by $11,125 in Public Health Realignment and $58,780 in Medi-Cal revenues.  The FY 2015-16 estimated costs is $209,707.  There is FY 2015-16 expected revenue of $33,371 in Public Health Realignment and $176,336 in Medi-Cal revenues.  The FY 2015-16 anticipated revenue is sufficient to cover increased costs.  Health will not exceed its allocation of General Fund Contribution.  There are sufficient appropriations in Health Department 4000-HEA003 and 4000-HEA004 Fiscal Year 2014-15 Adopted Budgets to accommodate this action
