RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.  Approve and authorize the Director of Health or the Assistant Director of Health to sign an Agreement with the Coalition of Homeless Service Providers (CHSP) in the amount of $160,000, to provide Management Analyst and Data Analyst staffing to develop cross-systems analysis and create a long-lasting homeless continuum of care data dashboard for use by policy makers, homeless service providers, potential volunteers, and the general public to provide greater homeless needs and awareness, transparency, and community engagement, for the term retroactive to January 1, 2021 through December 31, 2021; and

b.  Approve the recommendation of the Director of Health to accept non-standard auto insurance provisions in the Agreement; and

c.  Approve and authorize the Director of Health or the Assistant Director of Health to sign up to two (2) future amendments to this Agreement where the amendments do not significantly change the scope of work and do not cause an increase of more than ten percent (10%) (16,000) of the original contract amount.

 

SUMMARY/DISCUSSION:

The County of Monterey Health Department (“Health”) administers the Whole Person Care (WPC) pilot program that coordinates physical health, behavioral health, and social services for vulnerable Medi-Cal beneficiaries who are high users of multiple systems and continue to have or are at risk of poor health outcomes. The California Department of Health Care Services (CDHS) has extended the WPC pilot program another year, with a new sunset date of December 31, 2021. This 12-month WPC extension allows Health the opportunity to provide CHSP with CDHS funds to create a long-lasting homeless continuum of care data dashboard for use by policy makers, homeless service providers, potential volunteers, and the general public. Data sets displayed in lists, tables, and graphs may include:

 

                     Homeless services inventory and longitudinal analysis

                     Shelter bed occupancy rate stratified by persons served by service type and demographics (race/ethnicity, age groups, family compositions, chronic vs. intermittent homelessness, geographic jurisdiction, etc.)

                     Racial disparities analysis of shelter occupants

                     Statistics for people enrolled in Coordinated Entry

 

These datasets will also be incorporated into DataShare Monterey County, the Health Department’s interactive data platform.

 

The County’s total one-time cost of $80,000.00 will be matched dollar-for-dollar by CDHS under the WPC program, for a total County-CHSP Agreement amount of $160,000.00.  CHSP will leverage additional funds from other sources to fully provide for equipment, supplies, and associated indirect costs.

 

This request is retroactive to January 1, 2021 because the Department wanted assurance from the State that WPC funding would be extended to the end of calendar year 2021. The extension approval letter was provided on February 24, 2021.

 

This work supports the Monterey County Health Department 2018-2022 Strategic Plan Initiatives:  1. To empower the community to improve health and 2. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health Services. It also supports the following of the ten essential public health services, specifically:  1. Monitor health status to identify and solve community health problems.  3. Inform, educate, and empower people about health issues.  4. Mobilize community partnerships and action to identify and solve health problems, and 5. Develop policies and plans that support individual and community health efforts.  

 

OTHER AGENCY INVOLVEMENT:

The office of County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to legal form, and fiscal provisions, respectively.

 

FINANCING:

This Agreement is 100% funded by Whole Person Care grant funds.  Appropriations for this Agreement are included in the Health Department’s (HEA014-8438) FY 2020-21, Adopted Budget and will be included in its FY 20201-22, Requested Budget.
