a. Ratify and authorize the Contracts/Purchasing Officer to sign an Agreement with the County of Santa Clara in the amount of $20,000, for the period of July 1, 2014 to June 30, 2019, to provide confirmatory testing on specimens being tested for HIV, Syphilis, Blood Lead and other diseases or conditions for the Monterey County Health Department's Laboratory; and
b. Accept and approve the non-standard language and risk provisions; and
Authorize the Contracts/Purchasing Officer to approve up to three future amendments to this Agreement where the total amendment amounts do not exceed ten percent (10%) of the contract amount and do not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. SUMMARY/DISCUSSION:
This Standard Agreement is to obtain reference laboratory services from the County of Santa Clara's Public Health Laboratory (CSCLAB) for testing specimens for infectious diseases. The Monterey County Health Department's Laboratory (MCHD) utilizes CSCLAB to perform tests that are infrequently ordered (such as Syphilis serology, HIV serology and Blood Lead levels).
