a. Approve and authorize the Director of Health or the Assistant Director of Health to sign an Agreement with the California Department of Public Health, to provide Monterey County Health Department access to data through the California COVID-19 Outreach and Rapid Deployment (CORD) platform for a 10-year term beginning upon execution by both Parties; and
b. Approve and authorize the Director of Health or the Assistant Director of Health to sign up to three (3) future amendments to this Agreement where the amendments do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health or the Assistant Director of Health to sign an Agreement with the California Department of Public Health, to provide Monterey County Health Department access to data through the California COVID-19 Outreach and Rapid Deployment (CORD) platform for a 10-year term beginning upon execution by both Parties; and

SUMMARY/DISCUSSION:
California has developed a wide-range of tools to track COVID-19 prevention and mitigation efforts in the state. One of those tools is the CORD database which incorporates COVID-19 vaccine uptake data by census tract and makes this data available via agreement to partners, including local health departments. Data include the census tract, the city, the percentage of people who have been vaccinated, the percentage reached by partner's outreach, the Healthy Places Index score and quartile.
