Agreement No: A 10929
Approve and authorize the Contracts/Purchasing Officer to sign Amendment
No. 3 to the Mental Health Services Agreement No. A- 10929 with Allen/Loeb
Associates for the final evaluation reports to include the three 3)-month grant
extension for the Substance Abuse and Mental Health Services Administration
SAMHSA) Center for Substance Abuse Treatment CSAT) grant for
Homeless Program. Amendment No. 3 extends the term from Fiscal Year
2009-10 to October 31, 2010 and adds $8,075 for a revised total amount not to
exceed $437,746.
Upon motion of Supervisor Potter, seconded by Supervisor Armenta, and carried by those
members present, the Board hereby:
Approved and authorized the Contracts/Purchasing Officer to sign Amendment No. 3 to the
Mental Health Services Agreement No. A-10929 with Allen/Loeb Associates for the final
