a. Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) to execute the Fifth Amendment to the Professional Services Agreement (A-12234) with Danny Tan DDS to provide dental services to NMC patients, extending the term by twelve months and adding $45,000 for a revised total not to exceed amount of $300,000 for the full term of the Agreement, February 1, 2011 to June 30, 2016; and
b. Authorize the Deputy Purchasing Agent for NMC to execute to sign up to three (3) amendments to this agreement where the total amendments do not exceed 10% of the original contract amount, and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) to execute the Fifth Amendment to the Professional Services Agreement (A-12234) with Danny Tan DDS to provide dental services to NMC patients, extending the term by twelve months and adding $45,000 for a revised total not to exceed amount of $300,000 for the full term of the Agreement, February 1, 2011 to June 30, 2016; and

SUMMARY/DISCUSSION:
Danny Tan, DDS has provided dental services at his Salinas office to patients referred by Natividad Medical Center's NIDO (Natividad Immunology Division Outpatient) Clinic since 2008. The NIDO Clinic is a comprehensive primary care clinic that provides services, including referrals for dental services, to people over the age of 13 with HIV/AIDS and other infectious diseases. Such dental services are funded by the Ryan White Modernization Act Parts B & C grants.
