Agreement No: A-11194
a. Approve and authorize the Director of Health and Contracts/Purchasing
Officer to sign the Health Budget Unit 411 Professional Services
Agreement with BSK Analytical Laboratories in the amount of $225,000
for the period of July 1, 2008 through June 30, 2010, to provide analyti 1
laboratory services as required. by the Monterey County Heal h
Department; and
b. Authorize the Contracts/Purchasing Officer to sign up to three 3) futu e
amendments to this Agreement where the amendments do not exceed t n
percent 10%) of the total amount of the agreement and do n t
significantly change the scope of services.
Upon motion of Supervisor Salinas, seconded by Supervisor Armenta, and caied by those
members present, effective July 1, 2008, the Board hereby:
a. Approved and authorized the Director of Health and Contracts/Purchasin Officer to sign the
Health Budget Unit 411 Professional Services Agreement with BSK Ana ytical Laboratories
in the amount of $225,000 for the period of July 1, 2008 through June 3), 2010, to provide
analytical laboratory services as required by the Monterey County Health epartment; and
b. Authorized the Contracts/Purchasing Officer to sign up to three 3) future endments to this
Agreement where the amendments do not exceed ten percent 10%) of he total amount of
the agreement and do not significantly change the scope of services.
