a. Approve and authorize the Director of Health Services or designee to execute a non-standard Agreement with The Board of Trustees of the University of Illinois to receive training and a license to facilitate the Diabetes Empowerment Education Program (DEEP™), curriculum, for a term commencing upon execution and continuing for five years, for a total Agreement amount of $5,720;
b. Approve the recommendation of Director of Health Services to accept all non-standard risk provisions in Agreement; and
c. Approve and authorize the Director of Health Services or designee to sign up to three future amendments to this Agreement where the total amendments do not exceed 10% ($572) of the original Agreement amount, do not significantly alter the scope of work, and do not exceed the maximum amount of $6,292.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a.
