RECOMMENDATION:
It is recommended that the Board of Supervisors adopt a Resolution:

a. Accept the Certificate of Completion for Seismic Upgrade Project for Natividad Medical Center (NMC), located at 1441 Constitution Blvd., Salinas, CA 93906, Project No. 9600-27, Bid Package No. NMC-1001; and

b. Authorize the NMC Assistant Administrator of Operations & Support Services to execute and record the Notice of Completion.

SUMMARY/DISCUSSION:
On June 12, 2012 the Board of Supervisors awarded a construction contract to John F. Otto Inc. dba (Otto Construction) to complete the Seismic Upgrade Project for NMC. The Scope of the project was to bring new hospital buildings 500, 580A, and 980 on the first, second and third floors in compliance in the non-structural performance category (NPC-3) by bracing required for the fire sprinkler system at the sprinkler heads. Also, upgrades of buildings 600B and 940 at the second floor was limited to the emergency lighting and exit sign seismic bracing, this will bring these buildings into compliance with NPC-2.
The project started the week of July 16, 2012 and was completed by December 27, 2012. It was determined to have been satisfactorily performed in accordance with the plans and specification.
Otto Construction’s contract was in the amount of $306,850. There was an allowance of $30,000.00 included within Otto Construction’s general contract. Five change orders were issued and the total of $30,000.00 was used, Otto Construction’s contract was revised (increased) by $45,480.00 for a new contract amount of $352,330.
