Title
a. Accept a grant award of $150,000 (“Grant”) from Central California Alliance for Health (“Alliance”) in support of the development and implementation by the County of Monterey Health Department - Administrative Bureau (“Administrative Bureau”) of the 2025 Community Health Needs Assessment (“CHNA”) and the 2024-2027 Community Health Improvement Plan (“CHIP”) for Monterey County; and
b. d. Authorize the Director of Health Services or designee to approve up to 3 additional Amendments that do not to exceed 10% ($15,000) of the original Agreement amount, do not significantly alter the scope of services, and do not exceed the maximum amount of $165,000.

It is recommended that the County of Monterey Board of Supervisors:
a.
