a. Approve and authorize the Contracts/Purchasing Officer to sign Amendment No. 1 to the Agreement with Integra Telecom Holdings, Inc., to provide dedicated telecommunications circuits between the County and the Oregon Community Health Information Network, Inc., to increase the amount by $183,800 for a revised total not to exceed $194,411, and to extend the term to February 28, 2017; and
b. Authorize the Contracts/Purchasing Officer to approve up to three (3) future amendments to this Agreement where the amendments do not significantly change the scope of work or result in an increase to General Fund Contribution.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer to sign Amendment No. 1 to the Agreement with Integra Telecom Holdings, Inc., to provide dedicated telecommunications circuits between the County and the Oregon Community Health Information Network, Inc., to increase the amount by $183,800 for a revised total not to exceed $194,411, and to extend the term to February 28, 2017; and

SUMMARY/DISCUSSION:
The Monterey County Health Department, Clinic Services Bureau ("Clinic Services") entered into a three year agreement with Integra Telecom Holdings, Inc., beginning August 1, 2012 to July 31, 2015, in the amount not to exceed $45,000. The agreement is to support mission-critical telecommunication circuits between the County and the Oregon Community Health Information Network, Inc. (OCHIN), by providing Clinic Services 3Mbps of bandwidth to support clinic management functions such as email, web browsing and simultaneous use of Electronic Medical Records and real-time image downloads.
