b. Authorizing the Assistant Administrator of Operations & Support Services to execute and record the Notice of Completion. (AMENDMENT 1 AND NOTICE OF COMPLETION SUBMITTED VIA SUPPLEMENTAL)

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Accepting the Certificate of Completion for Outpatient Expansion 2nd Floor-Building 200 for Natividad Medical Center, located at 1441 Constitution Blvd., Salinas, CA 93906, Project No. 8842, Bid Package No. 10317; and
b. Authorizing the Assistant Administrator of Operations & Support Services to execute and record the Notice of Completion.
SUMMARY/DISCUSSION:
On December 12, 2011, the Board of Supervisors awarded a construction contract to Avila Brothers Construction, Inc, to complete the Outpatient Expansion 2nd Floor-Building 200 Project for Natividad Medical Center (NMC), Salinas, CA. Avila Brothers Construction, Inc’s work included: construction of 2,000 square feet of shell space, demolition of existing clinic space, new interior walls, millwork, finishes, ceiling work, and mechanical/electrical/plumbing work. The project started the week of January 2, 2012 and was completed on July 20, 2012. The work was performed satisfactorily in accordance with the plans and specifications.
Avila Brothers Construction Inc.’s contract was in the amount of $1,480,200.00. A construction contingency of 10% or $148,020.00 was recommended in order to ensure sufficient funds for completion of the project. Seven change orders were issued for a total of $127,396.49, increasing the total contract cost to $1,607,596.49. All work was accomplished within the contract amount and on schedule.
