a. Approve and authorize the Director of Health or Assistant Director of Health to execute a Mental Health Services Agreement with Sierra Vista Child and Family Services for the provision of residential services in the amount of $26,420 for Fiscal Year (FY) 2018-19, $155,916 for FY 2019-20, and $155,916 for FY 2020-21 for a total Agreement amount not to exceed $338,252 retroactive to April 30, 2019 through June 30, 2021; and
b. Approve and authorize the Director of Health or Assistant Director of Health to approve up to three (3) future amendments that do not exceed ten percent (10%) ($33,825.20) of the original Agreement amount and do not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. SUMMARY/DISCUSSION:
Sierra Vista Child and Family Services is a licensed Short-Term Residential Therapeutic Program (STRTP). The STRTP provides an integrated program of specialized and intensive care, services and supports, and 24-hour supervision on a short-term basis for children and youth with complex and severe needs.
