Title: a. Approve and authorize the Director or Assistant Director of the Department of Social Services to sign an agreement with Health Projects Center for $261,311, to provide Caregiver Support and Fall Prevention services to Monterey County individuals with disabilities, older adults and their care givers for the period of July 1, 2022 through June 30, 2023; and b. RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director or Assistant Director of the Department of Social Services to sign an agreement with Health Projects Center for $261,311, to provide Caregiver Support and Fall Prevention services to Monterey County individuals with disabilities, older adults and their care givers for the period of July 1, 2022 through June 30, 2023; and
b. Authorize the Director or Assistant Director of the Department of Social Services to sign up to three (3) amendments to the agreement, where the total amendments do not exceed 10% ($26,131) of the contract amount and do not significantly change the scope of work.

SUMMARY/DISCUSSION:
This agreement provides the following Family Caregiver Support and Fall Prevention services to older adults and individuals with disabilities in Monterey County:

- Caregiver counseling, which may range from guidance with caregiving responsibilities to therapy for stress, depression, and loss;

- Community education on caregiving designed to educate groups of current or potential caregivers and provide them with caregiver support resources and services;
