a. Approve and authorize the Director of Health Services or designee to execute a non-standard Agreement with Medbridge, Inc., to access a web-based system for the creation of home exercise programs for patients served in the California Children’s Services Medical Therapy Program, for a one-year term, effective upon execution, for a total amount not to exceed amount of $3,289;
b. Approve non-standard indemnification and insurance provisions as recommended by the Director of Health Services; and
c. Approve and authorize the County Purchasing Agent or designee to sign up to three future amendments to the agreement, provided they do not exceed an aggregate amount of $200,000 fall within the signing authority of the County Purchasing Agent, and do not significantly change the scope of services.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Director of Health Services or designee to execute a non-standard Agreement with Medbridge, Inc., to access a web-based system for the creation of home exercise programs for patients served in the California Children’s Services Medical Therapy Program, for a one-year term, effective upon execution, for a total amount not to exceed amount of $3,289;
