RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Receive an oral report from Monterey County Health Department on the 2014-2018 Community Health Improvement Plan; and
b. Authorize the distribution of final report to County departments, the public, and community partner organizations.

SUMMARY:
A presentation of highlights from the 2014-2018 Community Health Improvement Plan that describes strategies, stakeholders, goals, and measures of progress for specific health concerns and disparities, and encourages members of our local public health system to align priorities and actions in a highly focused and organized manner. Other objectives of compiling and sharing these data include supporting community empowerment and action, guiding program and agency work to reduce adverse social determinants of health, and fulfilling a prerequisite to national health department accreditation.

DISCUSSION:
This work supports Monterey County Health Department's 2011-2015 strategic plan initiatives to empower a network of health system partners to collectively impact and improve health through programs, policies, and activities. This work also supports four of the Centers for Disease Control and Prevention Ten Essential Public Health Services:
1. Monitor health status to identify and solve community health problems,
2. Inform, educate, and empower people about health issues,
3. Mobilize community partnerships and action to identify and solve health problems
4. Develop Policies & Plans that Support Individual & Community Health Efforts
Further, this assessment is a requirement in the Department's application for national local health department accreditation.
