a. Approve and authorize the Contracts/Purchasing Officer or designee to execute Amendment No. 3 to Agreement No. A-15021, pursuant to RFP #10742, with Firato Service Company, Inc., for specialized custodial maintenance services, to extend the term of the contract for one (1) additional year, for a revised full term of January 1, 2021 to December 31, 2024, and increase the contract amount by $450,000, for a revised contract amount not to exceed $1,547,749, and
b. Authorize the Contracts/Purchasing Officer or designee to sign up to three (3) future amendments to this Agreement where the amendments in total do not exceed 10% of the contract amount ($15,477) and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer or designee to execute Amendment No. 3 to Agreement No. A-15021, pursuant to RFP #10742, with Firato Service Company, Inc., for specialized custodial maintenance services, to extend the term of the contract for one (1) additional year, for a revised full term of January 1, 2021 to December 31, 2024, and increase the contract amount by $450,000, for a revised contract amount not to exceed $1,547,749, and

The Health Department Clinic Services Bureau (Clinic Services) operates ten community clinic sites designated as Federally Qualified Health Centers (FQHCs) which provide preventive, primary, and specialty medical care services.
