It is recommended that the Board of Supervisors:
Approve and authorize the Director of Health or Assistant Director of Health to execute Amendment No. 3 to Mental Health Services Agreement A-12513 with California Psychiatric Transitions, extending the term of the Agreement one year to June 30, 2017, and adding $47,775 for Fiscal Year (FY) 2015-16 and $1,367,010 for FY 2016-17, for a new total Agreement amount not to exceed $3,177,265.
SUMMARY/DISCUSSION
California Psychiatric Transitions (CPT), located in Delhi, California, is one of a very limited number of Mental Health Rehabilitation Centers (MHRC's) that specialize in working with people with severe mental disabilities. This facility provides client specific programs including behavioral adjustment, self-help skill development, pre-vocational preparation, and addressing special needs in interpersonal relationships. CPT provides specialized services at two primary care service levels that meet Behavioral Health clients' needs for immediate placement at a lower level of care upon hospital discharge. This facility is the last resort before a client is placed in a State Hospital.
This Amendment No. 3 to Agreement A-12513 with California Psychiatric Transitions extends the term of the Agreement from July 1, 2013 to June 30, 2016 for an additional year through June 30, 2017 for a new term of July 1, 2013 to June 30, 2017.
