Approve and authorize the Contracts/Purchasing Officer or Contracts/Purchasing Supervisor to sign an Amendment No. 2 to the Agreement with ISS Facility Services, Inc., on behalf of the Health Department to extend the term for an additional one (1) year, to June 30, 2018 for a revised term of July 1, 2014 to June 30, 2018, and to increase the amount by $639,271.80, for a revised contract amount not to exceed $1,166,948.66, for specialized custodial maintenance services as set forth in RFP No. 10484.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Contracts/Purchasing Officer or Contracts/Purchasing Supervisor to sign an Amendment No. 2 to the Agreement with ISS Facility Services, Inc. on behalf of the Health Department to extend the term for an additional one (1) year, to June 30, 2018 for a revised term of July 1, 2014 to June 30, 2018, and to increase the amount by $639,271.80, for a revised contract amount not to exceed $1,166,948.66, for specialized custodial maintenance services as set forth in RFP#10484.
SUMMARY/DISCUSSION:
Approval of the recommended action will maintain the essential, specialized custodial maintenance services and supplies currently provided to the Health Department's Clinics Services Bureau by ISS Facility Services, Inc. Service areas include exam rooms, waiting areas, offices, common areas, halls, bathrooms, and multi-purpose rooms as well as the planned expansions for Laurel Internal Medicine and Laurel Pediatric clinics. The Agreement is based upon County requirements set forth in the terms and conditions of RFP#10484.
The initial term commenced with the signing of the Agreement for a period of two (2) years, beginning July 1, 2014 through June 30, 2016 for an amount not to exceed $479,706.24 with the option to extend the Agreement for two (2) additional one (1) year periods.
