RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Director of the Department of Social and Employment Services (DSES), or his designee, to sign an agreement with Health Projects Center allowing the exchange of information to successfully transition the Multi-purpose Senior Services Program (MSSP) from Monterey County to Health Projects Center, effective February 1, 2012.

SUMMARY/DISCUSSION:
On June 30, 2011, DSES terminated its contract with the California Department of Aging to administer the Multipurpose Senior Services Program for Monterey County due to budgetary concerns. Health Projects Center of Santa Cruz (HPC) was selected by the California Department of Aging to administer the MSSP in Monterey County beginning February 1, 2012.

The purpose of the MSSP is to provide high quality social and health care management for frail elderly clients who are certifiable for placement in a nursing facility, but who wish to remain in the community. The Agreement between DSES and HPC allows for the exchange of needed information to transition the program, meet legal requirements and answer potential future requests for information about services provided. It will also assist in the enrollment of DSES’ former MSSP clients as priority into the HPC program. This Agreement was delayed coming to the Board due to extended negotiations with the vendor regarding the terms and conditions.

OTHER AGENCY INVOLVEMENT:
County Counsel reviewed and approved this Agreement.

FINANCING:
There are no fiscal implications associated with this Agreement.
