a. Approve and authorize the Director of Health or Assistant Director of Health to execute a two-year Standard Agreement with Fivebase, LLC for the provision of Avatar database technical and application consulting, and professional training services, for a maximum County obligation of $200,000 for the term of July 1, 2021 through June 30, 2023; and
b. Approve and authorize the Director of Health or Assistant Director of Health to approve up to three (3) future amendments that do not exceed ten percent (10%) ($20,000) of the original Agreement amount and do not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health or Assistant Director of Health to execute a two-year Standard Agreement with Fivebase, LLC for the provision of Avatar database technical and application consulting, and professional training services, for a maximum County obligation of $200,000 for the term of July 1, 2021 through June 30, 2023; and

Founded in 2009, Fivebase, LLC (formerly Virtulink, LLC) provides IT Consulting, Design, Integration, Configuration, and Support for small, medium and large for-profit and non-profit organizations and Governmental entities.
Monterey County Behavioral Health utilizes Avatar as the electronic health record system for the provision of behavioral healthcare services. Avatar is a Netsmart hosted solution and is not a standalone system but requires customization relative to the ancillary services and interfaces that exist in the vendor hosted solution.
