Title
a. Approve and authorize the Director of Health or designee to sign Agreement #14-10959 with the California Department of Public Health in the amount of $1,000,000, for the period January 1, 2015 to June 30, 2018, to provide services to local communities with populations at high risk for diabetes and cardiovascular disease under Domains 3 and 4, Health Systems Interventions of the 1305 Supplemental Funds in partnership with area safety net providers; and
b. Authorize the Director of Health or designee to approve three future amendments up to ten percent (10%) of the original contract amount, which does not significantly alter the scope of services; and
c. Authorize the Auditor-Controller to amend Fiscal Year 2014-15 Adopted Budget Fund 001, Appropriation Unit HEA014, Unit 8438 to increase revenue and appropriations in the amount of $250,000.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a.
