a. Approve and authorize the Director of Health Services or designee to execute Modification No. 1 to agreement No. A-15579 the Coordination Agreement for Cardiac Arrest Registry to Enhance Survival (CARES) Services for the provision of cardiac arrest data between the County of Monterey and the County of Sonoma for a period retroactive to June 30, 2026 through June 30, 2031 and replace Sections 4.1 (Term and Termination) and 7.6 (Notices) with updated Sections 4.1 and 7.6; and
b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future amendments that do not significantly change the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health Services or designee to execute Modification No. 1 to AGREEMENT No. A-15579 the Coordination Agreement for Cardiac Arrest Registry to Enhance Survival (CARES) Services for the provision of cardiac arrest data between the County of Monterey and the County of Sonoma for a period retroactive to June 30, 2026 through June 30, 2031 and replace Sections 4.1 (Term and Termination) and 7.6 (Notices) with updated Sections 4.1 and 7.6; and
b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future amendments that do not significantly change the scope of services.
SUMMARY:
The Director of Health Services seeks approval to execute Modification No. 1 to the Coordination Agreement for Cardiac Arrest Registry to Enhance Survival (CARES) Services with the County of Sonoma. This amendment extends the Agreement from June 30, 2026, to June 30, 2031. The amendment also replaces Section 4.1 and 7.6 with updated Sections 4.1 and 7.6.
