a. Approve and authorize the Director of Health to sign a three fiscal year term Mental Health Services Agreement (July 1, 2014 to June 30, 2017) with Beverly Health Rehabilitation Services, Inc. dba Golden Living Center San Jose for the provision of mental health services for Monterey County adult residents with severe psychiatric disabilities in the amount of $943,781 for Fiscal Year (FY) 2014-15, $986,866 for FY 2015-16, and $1,031,991 for FY 2016-17 for a total Agreement amount not to exceed $2,962,638; and
b. Authorize the Director of Health to sign up to three future amendments to this Agreement where the amendments do not exceed ten percent (10%) of the original Agreement amount and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. SUMMARY/DISCUSSION:
The County currently has Agreement A-11979 with Beverly Health Rehabilitation Services, Inc. dba Golden Living San Jose which will expire on June 30, 2014.
