RECOMMENDATION:
It is recommended that the Board of Supervisors authorize the Purchasing Manager for Natividad Medical Center (NMC) to execute the First Amendment to the Professional Services Agreement with Sweet, Renfer & Milanesa MD to provide urology services at NMC, extending the Agreement to June 30, 2013 and adding $270,000 for Fiscal Year 2013/2013 for a revised total Agreement amount not to exceed $540,000 in the aggregate.

SUMMARY/DISCUSSION:
Sweet, Renfer & Milanesa MD is a general partnership consisting of physicians that are licensed to practice medicine in the state of California. NMC currently has an agreement with Sweet, Renfer & Milanesa MD to provide 24 hours, 7 days a week urology call coverage in the emergency department as well as one urology clinic per week in the outpatient specialty clinic. NMC wishes to amend the agreement with Sweet, Renfer & Milanesa MD to extend the term for an additional twelve (12) months in order to continue to provide urology services to patients without interruption.

NMC has obtained an independent opinion of fair market value supporting the payment terms of this Agreement. The scope of services and compensation for FY 2012/2013 are the same as FY2011/2012. The additional $270,000 is equal the total compensation times the projected number of clinic days and call coverage that will be provided for the period July 1, 2012 to June 30, 2013.
