RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Contracts/Purchasing Officer to sign Amendment No. 6 to the Professional Services Agreement A-11565 with Medical Doctor Associates, LLC to provide Locum Tenens Physician services to extend the term from June 30, 2012 to June 30, 2015 and to increase the amount by $945,000 for a revised total not to exceed $2,134,000;  
 
SUMMARY/DISCUSSION:
Locum Tenens are licensed Physicians and Healthcare Professionals who are Board Certified or Board Eligible or have equivalent training and experience.  Locum Tenens provide temporary medical services in accordance with applicable Federal, State, and County laws, rules, regulations, and policies.  Medical Doctor Associates (MDA) is a Locum Tenens agency that provides physician health care placement services.  MDA has provided services to the County of Monterey under this Agreement since December 1, 2009.   
 
The County engages Physicians and Healthcare Professionals who provide Locum Tenens services in areas of critical medical need where sufficient permanent staffing is not available.  This amendment will assure the provision of essential mental health, medication support, primary care, and specialty care continues for clients of Monterey County Health Department's Behavioral Health and Clinic Services Bureaus during temporary physician absences.
  
Locum Tenens are hired on a per diem rate applicable to the professional medical specialty which has been negotiated with MDA.  The per diem rate is standard for this type of service and includes allowances for overtime, holiday, and on-call pay as listed in Amendment No 6, Exhibit A-6 Part 2:  Payment Provisions.
 
OTHER AGENCY INVOLVEMENT:
Amendment No. 6 to Agreement A-11565 has been approved by County Counsel as to legal form and Auditor-Controller as to fiscal provisions.  The Contract/Purchasing Manager concurs with this Amendment No. 6.  The insurance provisions remain unchanged from the original Agreement. Agreement A-11565 and subsequent Amendments are on file with the Clerk of the Board.
 
FINANCING:
Amendment No. 6 to Agreement A-11565 adds $315,000 per year for FY 2012-13, FY 2013-14 and FY 2014-15 for a total amount not to exceed $2,134,000.  Amendment No. 6 is financed by Realignment and Medicaid (Federal Financial Participation) funds for Behavioral Health Bureau and Medicaid for Clinic Services Bureau. Funding for this Amendment is included in the Health Department's Behavioral Health Bureau (4000-HEA012) and Clinic Services Bureau (4000-HEA007) Fiscal Year 2012-13 Requested Budgets.  Approval of this action does not impact General Fund Contribution.
