RECOMMENDATION:
It is recommended that the Board of Supervisors authorize the Purchasing Manager for Natividad Medical Center (NMC) to execute the First Amendment to the Professional Services Agreement with Juan Calzetta MD to provide vascular surgery services at NMC, extending the Agreement to June 30, 2013 and adding $213,000 for Fiscal Year 2013 for a revised total Agreement amount not to exceed $411,000 in the aggregate.
SUMMARY/DISCUSSION:
Juan Calzetta MD is licensed to practice medicine in the state of California and currently has an agreement with NMC to provide vascular surgery services. Dr. Calzetta provides 24/7 emergency department call coverage and a minimum of two vascular surgery clinics per week in the outpatient specialty clinic. NMC wishes to amend the current agreement with Dr. Calzetta in to continue to provide vascular surgery services to patients without interruption.
NMC has obtained an independent opinion of fair market value supporting the payment terms of this Agreement. The additional $213,000 is equal the total compensation for providing 24/7 call coverage and clinic for the period July 1, 2012 to June 30, 2013.
OTHER AGENCY INVOLVEMENT:
County Counsel has reviewed and approved this Agreement as to legal form and risk provisions. Auditor-Controller has reviewed and approved this Agreement as to fiscal provisions. The Agreement has also been reviewed and approved by Natividad Medical Center’s Board of Trustees.
FINANCING:
The total cost of this amended Agreement is not to exceed $411,000 in the aggregate and $213,000 is included in the fiscal year 2013 ...
