RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Director of Health to sign Amendment No. 6 to the Mental Health Services Agreement A-12268 with Door To Hope for the provision of mental health rehabilitation services.  Amendment No. 6 revises the Scope of Services, Payment and Billing Provisions and Budget and Expenditure Reports, and increases Fiscal Year (FY) 2014-15 annual amount by $432,904 for a revised total Agreement amount not to exceed $9,450,225 for FY 2012-13 through FY 2014-15.
 
SUMMARY/DISCUSSION:
Door to Hope (DTH) facilitates various treatment centers for alcoholism, drug abuse and mental health rehabilitation services throughout the City of Salinas.  This Agreement consists of six (6) program components:  Families First and Foremost (FFF), Integrated Co-occurring Treatment (ICT) ages 12 - 17 and effective April 1 ages 18 - 25, Monterey County Screening Team for Assessment, Referral and Treatment (MCSTART) ages 0 - 5 and MCSTART ages 6 - 11, Nueva Esperanza, and Santa Lucia.
 
Approval of this Amendment is mission critical to meet the requirements of the Mental Health Services Act (MHSA) and to sustain the continuity of core services to the children and their families in Monterey County.  The goal of each program is to improve the child's and youth's development, improve overall functioning as well as family functioning, and reduce the possibility of future residential care, out-of-home placement, and/or hospitalization.
 
This Amendment adds services for young adults ages 18 - 25 to the ICT program and adds $36,979 to the FFF services, adds $100,000 to ICT services, adds $200,000 to MCSTART ages 6 - 11 services, and removes the Door To Hope match amount of $95,925 thereby increasing the County's obligation by $95,925.  This Amendment 6 provides a total increase for the DTH programs in the amount of $432,904 for a contract amount of $3,756,041 for FY 2014-15.
The Agreement contains the County's standard 30-day "no cause" provision (Section IV. B) and an additional defunding provision (Section IV. D), which provides the County the ability to amend or terminate the Agreement in the event of a reduction and/or termination of funding.  
This work supports the Monterey County Health Department 2011-2015 strategic plan initiative: 1) Empower the community to improve health through programs, policies, and activities.  It also supports two of the ten essential public health services, specifically:  3) Inform, educate, and empower people about health issues, and 4) Mobilize community partnerships and action to identify and solve health problems.
 
OTHER AGENCY INVOLVEMENT:
County Counsel, Auditor-Controller, and Risk Management have reviewed and approved this Amendment as to legal form, fiscal provisions, and insurance requirements.  Amendment No. 6 to Agreement A-12268 with Door to Hope is on file with the Clerk of the Board.
 
FINANCING:
This Amendment is funded by Medi-Cal (74%) and Mental Health Services Act (MHSA) (26%) revenues.  The funds for this Amendment adding $432,000 are included in the Health Department's Behavioral Health (HEA012, Unit 8410) Fiscal Year 2014-15 Adopted Budget.
