RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Authorize Deputy Purchasing Agent for Natividad Medical Center (NMC) to execute the First Amendment to the Professional Services Agreement with Kyle Youngflesh D.O. to provide family medicine services at NMC effective September 1, 2015, extending the term and adding $100,000 for a revised Agreement amount not to exceed $200,000 for the full term of the Agreement November 1, 2014 to December 31, 2016.

SUMMARY/DISCUSSION:
NMC entered into an agreement on November 1, 2014 with Kyle Youngflesh D.O. to provide family medicine services including rounding on hospitalized patients on the medical surgical unit. Shortly after Dr. Youngflesh started working at NMC, the Family Medicine service had a need for additional staff to see patients in the family medicine clinic. The maximum liability was nearly reached in the first six months of the agreement due to the need to cover for a vacant Hospitalist position and the additional need for coverage in Family Medicine. NMC wishes to amend the agreement with Dr. Youngflesh add family medicine clinic services to the scope of services; extend the term and add to the amount payable for services provided under this contract.

Dr. Youngflesh is paid an hourly rate according to the terms of the Agreement. The amount of services may increase due to patient volume and need for services, but the rate of pay remains the same. NMC has obtained an independent opinion of fair market value supporting the payment terms of this Agreement.
