RECOMMENDATION:

 

It is recommended the Board of Supervisors:

 

a.                     Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) to execute Amendment No. 5 to the Agreement (A-12807) with Healthcare Transformation, LLC (HCT) for temporary management and consulting services at NMC, extending the term of the Agreement an additional thirteen (13) month period for a revised full Agreement term of January 1, 2014 through March 31, 2016 and adding $392,800 for a revised total Agreement amount not to exceed $1,020,600.

 

SUMMARY/DISCUSSION:

 

Natividad Medical Center (NMC) entered into an agreement with Healthcare Transformation, LLC (“HCT”), a national firm experienced in providing healthcare consulting services and interim professional placements, earlier this year for specialized consulting services to evaluate NMC's current Core Measure performance and processes and to implement best practices to improve performance.  It is necessary to amend this Agreement to extend the Compliance Officer position until permanently filled, to add an Emergency Department Nursing Director position until permanently filled, and to include consulting services for NMC’s multi-specialty outpatient clinic.

 

The position of Risk and Compliance Officer has been filled by an Interim Appointee for several years.  The evolving complexity of the regulatory environment specific to health care has created a need to once again fill this position. A search is presently underway for a Chief Compliance Officer though Natividad Medical Center has engaged a Compliance Consultant through HCT to review and evaluate our program prior to the selection of a permanent candidate. This consultant began her engagement on February 2, 2015 and continues to provide NMC with compliance officer services including measurable deliverables. Natividad Leadership in consultation with other County departments deemed this position to be urgently needed.

 

Natividad Medical Center has experienced rapid growth in outpatient specialty services. Last fiscal year our specialty clinics saw over 2000 visits per month with the addition of outpatient clinic services in neurosurgery, trauma and orthopedics. NMC has identified a consultant with extensive management experience in multispecialty ambulatory networks. This consultant will assess current operations and will develop and assist with the implementation of recommendations regarding staffing, patient flow, and customer service in the clinic.

 

The Emergency Department (ED) Nursing Director position has been filled by an Interim appointee who also provides nursing leadership to two other departments. Our ED volume has grown over the past year and NMC believes that dedicated leadership will assist with staff management and employee engagement.

 

Healthcare Transformation, LLC (“HCT”) is a national firm experienced in providing interim management level professional placements in healthcare organizations.

 

OTHER AGENCY INVOLVEMENT:

 

County Counsel has reviewed and approved this Amendment No. 5 as to legal form and risk provisions, and the Auditor-Controller has reviewed and approved as to payment provisions. The Amendment No. 5 has also been reviewed and approved by NMC’s Finance Committee on 7/23/15 and by its Board of Trustees on 8/7/15.

 

FINANCING:

 

The actual maximum cost for Amendment No. 5 is $601,955 but the requested amount for Amendment No. 5 is only $392,800  because there was $171,400 previously approved by the Board of Supervisors for a Chief Information Officer position which was never utilized by NMC so that amount can be applied to costs going forward, and $42,000 of the $176,800 previously approved for the Infection Preventist position was unspent and can also be applied to offset the requested amount needed for Amendment No. 5. $601,955 has been included in the Fiscal Year 2015-16 Adopted Budget for this Agreement. There is no impact to the General Fund.
