Title: a. RECOMMENDATION:

 

It is recommended the Board of Supervisors:

 

a.                     Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) or his designee to execute a Master Agreement with CareFusion Solutions, LLC (CareFusion) for the provision of Medmined software used for surveillance, analytics, benchmarking, alerting and reporting in medication management and infection control, including a Customer Order, and containing non-standard risk terms regarding limitation on liability, non-standard insurance provisions, limitation on warranty and indemnification, in an amount not to exceed $288,904 for a 5-year term of March 23, 2016 through March 22, 2021; and

b.                     Authorize the Deputy Purchasing Agent for NMC or his designee to execute up to three (3) future amendments to the Agreement, which do not significantly alter the scope of work and do not cause an increase of more than ten percent ($28,890) of the original cost of the Agreement per each amendment.

 

SUMMARY/DISCUSSION:

 

CareFusion’s product, Medmined, is surveillance, analytics, benchmarking, alerting and reporting software for use in medication management and infection control. The software takes multiple data feeds from the hospital information system, combines the data, applies rules to the data, and then presents relevant data to the clinician. Medmined replaces several manual processes decreasing the amount of time needed to perform those tasks. This enables clinicians to focus on additional activities or in the education of their staff. Medmined provides near real time data analytics bringing relevant data to the clinician, allowing interventions to happen closer to the event. 

 

Healthcare associated infections (HAIs) are infections acquired in the hospital during treatment for another condition. Several of these are reportable to governmental agencies. HAIs cost hospitals money and hurt patients. HAIs can be preventable if certain procedures are followed. Medmined helps the infection preventionist to identify potential HAIs, identify potential procedure compliance issues with individual healthcare providers or units, and automates the required reporting to governmental agencies. This provides the infection preventionist more time to educate staff on procedures, and information to better determine specific areas to focus their attention.

 

Pharmacists monitor patients closely who are taking certain medications. Some medications have a narrow window of effectiveness with ineffective treatment at lower blood levels and toxicity at higher blood levels. Other medications may cause adverse reactions that if they occur require the medication to be stopped. Medication levels and the patient’s reaction to these medications are reviewed on a daily basis. The Medmined software will present the pharmacist only data that is actionable. Pharmacists will no longer spend inordinate amounts of time reviewing items that are in range; this will allow time for pharmacists to monitor a higher number of medications. In addition, the software allows for easy documentation of pharmacist actions for later reporting to hospital committees.

 

In 2014, under California Senate Bill 1311, California became the first state in the country to require that all hospitals implement an antimicrobial stewardship policy and antimicrobial stewardship committee. The California Department of Public Health followed up with eleven (11) activities that are necessary for an antimicrobial stewardship program.  Medmined will assist the committee with three (3) of these requirements.

 

OTHER AGENCY INVOLVEMENT:

 

County Counsel has reviewed and approved this Agreement as to legal form, and reviewed for risk provisions, and the Auditor-Controller has reviewed and approved as to payment provisions. The Agreement was reviewed by NMC’s Finance Committee on January 4, 2016 and Board of Trustees on January 8, 2016; however due to an error in calculation of the total amount of the Agreement, this item was re-reviewed by NMC’s Finance Committee on February 25, 2016 and Board of Trustees on March 4, 2016.

 

FINANCING:

 

The total cost for this Agreement is not to exceed $288,904.  CareFusion will not invoice until Fiscal Year 2016-17.  Funding for all future years of the Agreement will be included in those budgets, as appropriate. There is no impact to the General Fund.
