a. Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) or his designee to execute Amendment No. 1, including three Customer Orders, to the Master Agreement A-13080 with CareFusion Solutions, LLC (CareFusion) originally executed for Medmined software, to add a new software product, Alaris (IV pumps for patients), adding an additional $1,245,525 for a revised total Agreement amount not to exceed $1,534,429, and to extend the original of term of March 23, 2016 through March 22, 2021 by an additional four months for a revised Agreement term of March 23, 2016 through July 31, 2021; and
b. Authorize the Deputy Purchasing Agent for NMC or his designee to execute up to three (3) future Amendments to the Agreement, which do not significantly alter the scope of work and do not cause an increase of more than ten percent ($28,890) of the original cost of the Agreement per each Amendment.

RECOMMENDATION:
It is recommended the Board of Supervisors:
a. Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) or his designee to execute Amendment No. 1, including three Customer Orders, to the Master Agreement A-13080 with CareFusion Solutions, LLC (CareFusion) originally executed for Medmined software, to add a new software product, Alaris (IV pumps for patients), adding an additional $1,245,525 for a revised total Agreement amount not to exceed $1,534,429, and to extend the original of term of March 23, 2016 through March 22, 2021 by an additional four months for a revised Agreement term of March 23, 2016 through July 31, 2021; and
b. Authorize the Deputy Purchasing Agent for NMC or his designee to execute up to three (3) future Amendments to the Agreement, which do not significantly alter the scope of work and do not cause an increase of more than ten percent ($28,890) of the original cost of the Agreement per each Amendment.
SUMMARY/DISCUSSION:
On March 22, 2016 the Board of Supervisors approved Master Agreement A-13080 and Customer Order in the amount of $288,904 between Natividad Medical Center (NMC) and CareFusion Solutions LLC for the provision of Medmined software used for surveillance, analytics, benchmarking, alerting and reporting in medication management and infection control. NMC desires to amend the Master Agreement to add three Customer Orders for the Alaris product and services. The Master Agreement contains previously agreed upon terms and conditions by both parties. NMC desires to acquire the Alaris product as described below.
Alaris Product: An intravenous infusion pump (IV pump) is used to place fluids and medications directly into a patient’s blood stream. Modern IV pumps contain software that allows pumps to be programmed by nurses and physicians to infuse medications at different rates. The software contains libraries of different medications and their infusion parameters. These pumps ensure safety by preventing medications from being infused at rates greater than or less than safe limits. The bulk of the current IV pumps was first purchased in 2007, and has become outdated due to numerous issues. These pumps are no longer available from the vendor, lack the ability to bolus medications, and are not capable of being updated wirelessly. Updating the pump library for these pumps is time and labor intensive requiring that every pump be located and updated one at a time. Pumps not found will not be updated and may present a patient safety hazard.
The newest infusion pump safety feature in development is integration with hospital information systems. This is bidirectional with information going both to and from the pump. Information going to the pump is used for pump auto-programming, eliminating the need for a user to input information to program the pump. Information leaving the pump flows into the hospital information system for automatic documentation. This eliminates the need for the user to transcribe infusion times, titration information and volume infused. These changes save nurses time, improve documentation, and promote patient safety. The hospital information system vendor does not currently provide pump integration but is working on support in its next platform.
The Alaris IV pump allows for wireless updating of the library and bolus dosing. It is used by the majority of the hospitals in the area, and uses the same software and user interface that our Patient Controlled Analgesia pumps use. It has the greatest number of hospitals utilizing auto-programming and auto-documentation features of any vendor. While the hospital’s information system does not currently support these features, the Alaris pump puts Natividad in the best position to implement these features when available. Purchasing the Alaris pumps would allow Natividad to standardize on one format, improve safety with wireless upgradability and position itself for IV pump integration in the future.
Implementation of Alaris is scheduled to begin on September 13, 2016 as per the attached Implementation Schedule.
Negotiations were lengthy and ultimately the contract contains non-standard risk terms regarding limitation of liability, non-standard insurance provisions, limitation on warranty and indemnification.
OTHER AGENCY INVOLVEMENT:
County Counsel has reviewed and approved this Agreement as to legal form, and reviewed for risk provisions, and the Auditor-Controller has reviewed and approved as to payment provisions. The Agreement was reviewed by NMC’s Finance Committee on June 23, 2016 and Board of Trustees on June 27, 2016.
FINANCING:
The total cost for this Amendment No. 1 is $1,245,525, of which all was included in the Fiscal Year 2016-17 Adopted Budget. There is no impact to the General Fund.
