RECOMMENDATION:

 

It is recommended that the Board of Supervisors:

a. Accept a Grant Award from Central California Alliance for Health (CCAH) in support of a Patient Centered Medical Home practice coaching engagement by a CCAH-contracted consultant (“Engagement”) at Natividad Medical Group, a hospital-based primary care clinic of Natividad Medical Center (“NMC”).

b. Authorize the Deputy Purchasing Agent for NMC or his designee to execute an Agreement for Medi-Cal Capacity Grant, Practice Coaching Program Grant Number 0616-NMC-PC1, for the provision of the Engagement, effective as of the Grant Effective Date and terminating one or two years after the Grant Effective Date or upon completion of the engagement and submission of the final report by NMC.

 

SUMMARY/DISCUSSION:

 

Central California Alliance for Health (CCAH) established the Medi-Cal Capacity Grant Program to offer grants to health care providers and community organizations to support efforts that advance the mission of CCAH to provide timely access to quality health care services and to increase Medi-Cal capacity in the service area. 

 

CCAH has awarded the Practice Coaching Program Grant to the County of Monterey on behalf of Natividad Medical Center which provides NMC with one consulting engagement that will be conducted by Qualis Health or another CCAH-contracted consultant. The purpose of this Grant is to allow NMC to engage in patient centered medical home practice coaching with Qualis healthcare so as to foster and promote the mission of the Central California Alliance for Health (CCAH)’s Medi-Cal Capacity Grant Program. There are no match fund requirements associated with this grant.

 

OTHER AGENCY INVOLVEMENT:

 

County Counsel has reviewed and approved the agreement as to form and risk. The County Auditor-Controller has reviewed and approved the agreement. NMC did not have ample time to submit the grant agreement for review by the NMC Finance Committee and by NMC Board of Trustees due to an October 19, 2016 deadline by CCAH to return the grant agreement signed. 

 

FINANCING:

 

Pursuant to the Grant Award, the County has been allocated grant funding for the provision at NMC’s Natividad Medical Group of one CCAH Patient Centered Medical Home practice coaching engagement by a CCAH-contracted consultant.  The County does not receive the allocated funds under this Grant.  Instead, the funds are paid by CCAH directly to the CCAH consultant following the Engagement, pursuant to a separate agreement between CCAH and the consultant.
