RECOMMENDATION:
It is recommended that the Board of Supervisors:
Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) or his designee to execute the Fourth Amendment to Professional Services Agreement with Jose Mario Pauda M.D. to provide family medicine services, extending the term of the agreement by twenty-four (24) months (July 30, 2017 to June 30, 2019) for revised full agreement term of July 1, 2011 to June 30, 2019, and adding $40,000 for a revised total not to exceed amount of $140,000 in the aggregate.
SUMMARY/DISCUSSION:
NMC operates the Natividad Immunology Division Outpatient (NIDO) Clinic, a comprehensive primary care clinic serving adults with HIV/AIDS and other infectious diseases. NMC has an agreement with Jose Mario Pauda MD to provide family medicine services in the NIDO clinic one half day per week. NMC wishes to amend the agreement with Dr. Pauda to extend the term an additional twenty-four (24) months in order to continue to provide services without interruption. Dr. Pauda is paid an hourly rate according to the terms of the Agreement, although the amount services provided may increase due to patient volume, the rate of pay remains the same. NMC has obtained an independent opinion of fair market value supporting the payment terms of this Agreement.
OTHER AGENCY INVOLVEMENT:
County Counsel has reviewed and approved this Amendment as to legal form and risk provisions. Auditor-Controller has reviewed and approved this Amendment as to fiscal provisions. The Amendment has also been reviewed and approved by Natividad Medical Center’s Finance Committee and Board of Trustees on May 5, 2017.
FINANCING:
The cost for this Amendment is $40,000. The total not to exceed amount of this Agreement is $140,000. $16,014 was disbursed in Fiscal Year 2011/2012; $15,706 was disbursed in Fiscal Year 2012/2013; $15,712 was disbursed in 2013/2014; $15,712 was disbursed in Fiscal Year 2014/2015; $16,026 was disbursed in Fiscal Year 2015/2016; and $12,246 was disbursed in Fiscal Year 2016/2017 (9 months). $20,000 is included in the Fiscal Year 2017/2018 Recommended Budget. The remaining amount will be budgeted in subsequent years. There is no impact to the General Fund.

The services rendered in this agreement provide NMC with the additional support it needs in order to provide reliable and high quality patient care which improves the health and quality of life for patients and their families.
