RECOMMENDATION:
It is recommended that the Board of Supervisors:
Authorize the Contracts/Purchasing Officer to sign Amendment No. 4 to 340B Prime Vendor Participation Agreement Nos. A-12009 and A-12010 with Cardinal Health 110, Inc. and Cardinal Health 411, Inc. for the purchase of discounted outpatient pharmaceuticals for the Federally-Qualified Health Center Look-alike clinics, with a shipping location at the Alisal Health Center, extending the term one (1) additional year, for a revised term of March 24, 2011 to June 30, 2018.

SUMMARY/DISCUSSION:
The Health Department Clinic Services Bureau (Clinic Services) operates nine community clinic sites designated as Federally Qualified Health Center Look-Alikes (FQHC-LA). Under the direction of the County and governance of the Community Health Center Board, the clinics provide preventative, primary, and specialty medical care services.

On June 21, 2011, the Board of Supervisors approved Agreement Nos. A-12009 and A-12010 with Cardinal, enabling Clinic Services FQHC-LA clinics, pursuant to Section 340B of the Federal Public Health Services Act, to purchase outpatient drugs for eligible patients at discounted rates from drug manufacturers that enter into drug purchasing agreements with the United States Health Resources and Services Administration (HRSA).

The initial term of the Agreement was March 24, 2011 to June 30, 2013 and, since then, has been amended through Amendment Nos. 1, 2 and 3 to extend the term and modify the facility list. ...
