Report

RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.                     Approve and authorize the Director of Health or Assistant Director of Health to execute a one-year MHS Agreement with California Psychiatric Transitions for the provision of mental health services for adults with severe psychiatric disabilities, for a maximum County obligation of $2,064,645 retroactive to July 1, 2017 through June 30, 2018; and

b.                     Approve and authorize the Director of Health or Assistant Director of Health to approve up to three (3) future amendments that do not exceed ten percent (10%) ($206,465) of the original Agreement amount and do not significantly alter the scope of services.

 

SUMMARY/DISCUSSION:

California Psychiatric Transitions (CPT), located in Delhi, California, is one of a very limited number of Mental Health Rehabilitation Centers (MHRC’s) that specialize in working with people with severe mental disabilities.  This facility provides client specific programs including behavioral adjustment, self-help skill development, pre-vocational preparation, and addressing special needs in interpersonal relationships.  CPT provides specialized services that meet Behavioral Health clients’ needs for immediate MHRC placement at a lower level of care upon hospital discharge, as well as court-ordered diversion hold levels for individuals found both competent and incompetent to stand trial.  CPT also provides intensive therapy for patients struggling with disruptive behaviors, including assaultive and self-harming behaviors, with a higher resident to staff ratio, and 24-hour nursing.  This facility is the last resort before a client is placed in a State Hospital.

 

The County currently maintains Agreement A-12513 with California Psychiatric Transitions which will expire on June 30, 2017.  Based on an increased demand for the high-quality care CPT provides, this Agreement increases Units of Service across all three service levels, MHRC main floor, Diversion, and the Disruptive Behavioral Unit.  The approval of this Agreement will provide the necessary resources and increase the options for clients to be placed in a facility that best suits their needs and to live in the least restrictive community setting as possible considering their disabilities.  This Agreement meets the requirements of the State Welfare and Institutions Code 5600 (Bronzan-McCorquodale Act), the California Code of Regulations Titles 9 and 22, and sustains the continuity of care services to seriously mentally ill clients of Monterey County.

 

This Agreement is delayed in being brought before your Board due to a backlog of Contracts terming in FY 2016-17 that required renewal in the February-June 2017 timeframe.

This Agreement contains the County’s standard 30-day “no cause” provision (Section IV, B) and an additional defunding provision (Section IV, D), which provides the County the ability to amend or terminate the Agreement in the event of a reduction and/or termination of funding.

 

This work supports the following Monterey County Health Department 2011-2015 strategic plan initiative: 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports one of the ten essential public health services, specifically: 7) Link people to needed personal health services and assure the provisions of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

County Counsel and Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions, respectively.

 

FINANCING:

This Agreement is 100% funded by 1991 Mental Health Realignment revenues.  The funds for this Agreement ($2,064,645) are included in the Health Department’s Behavioral Health (HEA012, Unit 8410) Fiscal Year 2017-18 Adopted Budget.
