a. Approve and authorize the Director of Health or Assistant Director of Health to execute a two-year Facility Use Agreement for the term September 1, 2017 to August 31, 2019 with Community Health Innovations, LLC. (“CHI”) to use office space on County-leased premises located in Seaside, Marina and Salinas, California to provide Pediatric Wellness Program sessions to eligible Clinic Services’ patients at no cost; and
b. Approve and authorize the Director of Health or the Assistant Director of Health to sign up to three (3) future amendments to this Agreement where the amendments do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health or Assistant Director of Health to execute a two-year Facility Use Agreement for the term September 1, 2017 to August 31, 2019 with Community Health Innovations, LLC. (“CHI”) to use office space on County-leased premises located in Seaside, Marina and Salinas, California to provide Pediatric Wellness Program sessions to eligible Clinic Services’ patients at no cost; and
b. Approve and authorize the Director of Health or the Assistant Director of Health to sign up to three (3) future amendments to this Agreement where the amendments do not significantly change the scope of work.
SUMMARY/DISCUSSION:
The Health Department Clinic Services Bureau (Clinic Services) operates nine community clinic sites designated as Federally Qualified Health Center Look-Alikes (FQHC-LA). Under the direction of the County and governance of the Community Health Center Board, the clinics provide preventative, primary, and specialty medical care services.
CHI’s Pediatric Wellness Program offers free pediatric wellness coaching services through group education and one-on-one sessions for families referred by their child’s pediatrician. The six (6) week curriculum includes group learning on goal-setting, healthy eating and physical activity. Sessions are facilitated by a pediatric wellness coach with the primary goal of helping families create lifelong healthy habits. The County of Monterey has suitable clinic space and equipment to support the Pediatric Wellness Program and is willing to allow CHI to utilize such facilities to educate our patient population about nutrition and creating healthy lifestyle habits.
Approval of the recommended action will (1) allow CHI to begin offering the program directly to our patient population in locations that are familiar and accessible to them, and (2) directly affect County of Monterey's efforts to promote a healthy lifestyle to better prevent and reduce the risk of obesity and diabetes in our community.
This work supports the Monterey County Health Department 2011 - 2015 Strategic Plan initiative: 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one or more of the ten essential public health services, specifically: 3) Inform, educate and empower people about health issues, 4) Mobilize community partnerships and action to identify and solve health problems, and 7) Link people to needed personal health services and assure the provision of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
County Counsel and the Auditor-Controller have reviewed and approved the Agreement. Risk has reviewed and accepts the non-standard provisions.
FINANCING:
There are no fiscal provisions included as part of this Agreement.
