RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.                     Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) or his designee to execute the First Amendment to Professional Services Agreement with Candice Becker-Schwarm CNM to provide nurse midwife services, extending the term by twenty-four (24) months (October 1, 2017 to September 30, 2019) for a revised full agreement term of January 1, 2017 to September 30, 2019 and adding $350,000 for a revised amount not to exceed $450,000 in the aggregate; and

b.                     Authorize the Deputy Purchasing Agent for NMC to sign up to three (3) future amendments to this agreement where the total amendments do not exceed the term by more than twelve (12) months, do not significantly change the scope of work, and do not cause an increase of more than ten percent 10% ($10,000) of the original contract amount per each amendment.

 

SUMMARY/DISCUSSION:

Natividad Medical Center operates a labor and delivery unit with over 2,000 deliveries per year.  In January 2017, NMC added midwifery services to enhance the breadth of its obstetric and gynecological services becoming the only hospital in Monterey County to offer this service to its patients. A midwife is a trained health professional who helps healthy women during labor, delivery and after the birth of their babies.

 

NMC has an agreement with Candice Becker-Schwarm, a certified nurse midwife (CNM) to provide one half day of outpatient clinic per week and at least one 9 hour or 24 hour shift rounding on inpatients on the labor and deliver unit per week. 

 

NMC wishes to amend the agreement before its natural term to add funds and to extend the term by twenty-four months so that Candice can continue to provide at least the same services without interruption.  Candice is paid an hourly rate according to the terms of the agreement, although the amount of services provided may increase due to the need for coverage, the rate of pay remains the same.   NMC has obtained an independent opinion of fair market value supporting the payment terms. 

 

 

 

OTHER AGENCY INVOLVEMENT:

County Counsel has reviewed and approved this Amendment as to legal form and risk provisions.  Auditor-Controller has reviewed and approved this Amendment as to fiscal provisions.  The Amendment has also been reviewed and approved by Natividad Medical Center’s Finance Committee on August 24, 2017and Board of Trustees on September 8, 2017.

 

FINANCING:

The cost of this Amendment is $350,000.  The total not to exceed amount of the Agreement is $450,000 for the period January 1, 2017 to September 30, 2019.  $64,052 was disbursed in Fiscal Year 2016/2017 (6 months).  $175,000 is included in the Fiscal Year 2017/2018 Adopted Budget.  The remaining amount will be budgeted for subsequent fiscal years. There is no impact to the General Fund.

The services rendered in this agreement provide NMC with the additional support it needs in order to provide reliable and high quality patient care which improves the health and quality of life for patients and their families.
